About This Role
Tile Health empowers health systems to move beyond reactive care delivery toward proactive population health management. The Population Health Manager will work with partner organizations to deploy risk stratification models, close care gaps, and measure the impact of interventions on clinical and financial outcomes. This is a high-visibility role that combines clinical domain knowledge with data fluency and operational leadership.
What You'll Do
- Implement and manage population health stratification workflows using Tile Health’s analytics platform for partner health systems
- Collaborate with clinical and quality teams at partner sites to design care gap closure campaigns targeting chronic disease management
- Analyze claims, ADT, and clinical data to identify high-risk patient cohorts and quantify intervention impact
- Develop dashboards and reporting packages that track population-level quality measures such as HEDIS, Stars, and ACO benchmarks
- Facilitate cross-functional working sessions between care management, pharmacy, and social work teams to coordinate outreach
- Present quarterly outcomes reports to partner executives and Tile Health leadership
What We're Looking For
- 5+ years of experience in population health management, care management, or health plan quality improvement
- Strong working knowledge of risk adjustment methodologies (HCC, CDPS) and quality measurement frameworks (HEDIS, CMS Stars)
- Proficiency in health data analytics tools and the ability to interpret complex clinical datasets
- Experience partnering with ACOs, health plans, or risk-bearing provider organizations
- Excellent project management and stakeholder facilitation skills
Nice to Have
- MPH, MHA, or clinical degree (RN, NP, PA)
- Experience with social determinants of health screening and referral workflows
- Background working with Medicaid or Medicare Advantage populations
- Familiarity with community health worker programs and behavioral health integration models
