Nurse Care Manager

About This Role

Tile Health is expanding its clinical team to support rapid growth in enrolled patients across primary care partners. As a Nurse Care Manager, you will be the trusted voice patients hear each month, ensuring care plans stay current and escalations reach the right provider quickly.

What You'll Do

• Conduct monthly telephonic check-ins with enrolled chronic care patients and document every interaction for billing compliance
• Maintain and update individualized care plans in coordination with primary care providers
• Identify clinical red flags and escalate to on-call providers following established protocols
• Partner with the enrollment team to onboard newly eligible patients into the program

What We're Looking For

• Active RN license in good standing with the ability to obtain compact licensure
• 3+ years of nursing experience, ideally in care coordination or ambulatory care
• Strong telephonic communication skills and comfort with EHR documentation
• Working knowledge of chronic conditions such as diabetes, CHF, and COPD

Nice to Have

• Experience with Medicare chronic care management or APCM programs
• Bilingual English and Spanish
