About This Role
Tile Health’s value proposition depends on deep, reliable integrations with the EHR systems where clinicians spend their working day. The Backend Engineer for EHR Integration will design and implement APIs, webhook handlers, and data synchronization services that connect our platform with Epic, Cerner, and other leading EHRs. You will tackle the unique challenges of healthcare interoperability-inconsistent data formats, varying API capabilities, and strict security requirements.
What You'll Do
- Design and implement RESTful and FHIR-based APIs that facilitate bidirectional data exchange with Epic, Cerner, and other EHR systems
- Build robust message processing services for HL7v2 ADT, ORU, and SIU feeds with comprehensive error handling and retry logic
- Develop and maintain EHR-embedded applications (SMART on FHIR) that surface Tile Health insights within provider workflows
- Architect data synchronization patterns that maintain eventual consistency between Tile Health’s data store and source EHR systems
- Write comprehensive integration tests that simulate partner EHR behavior across different software versions and configurations
What We're Looking For
- 4+ years of backend software engineering experience with production-scale distributed systems
- Strong proficiency in TypeScript or Python with experience building RESTful API services
- Working knowledge of healthcare interoperability standards including HL7 FHIR R4, HL7v2, and SMART on FHIR
- Experience with message queue systems (Kafka, RabbitMQ, or SQS) for asynchronous processing
- Familiarity with relational databases (PostgreSQL) and document stores in a multi-tenant environment
- Understanding of HIPAA technical safeguards and secure coding practices for protected health information
Nice to Have
- Direct experience with Epic App Orchard or Cerner Code program certification processes
- Familiarity with Mirth Connect or Rhapsody integration engines
- Experience building OAuth 2.0 and OpenID Connect authentication flows for clinical applications
